Doktor Drauzia Varella byl povolán do největší věznice v Brazílii - Carandiru, aby zahájil prevenci a léčbu postižených virem HIV. Dostal se do prostředí jedné z nejdrsnějších věznic na světě, kde se přímo setkal s absolutně nelidským zacházením s vězni. Stovky vězňů žili v hrozných podmínkách. V roce 1992 ve věznici Carandiru vypukla série nepokojů, které přerostli do vzpoury, při které přišlo o život 111 vězňů. Film je inspirovaný skutečnými událostmi, které popsal doktor Drauzia Varella ve své knize Carandiru Station.
