Året er 2056. Menneskeheden er tæt på at bukke under for en uhyggelig organ-virus, men ud af håbløsheden træder firmaet GeneCo. GeneCo tilbyder organtransplantationer til de syge. Men operationerne er dyre – og hvis man ikke kan betale, opsøger de dystre Repo-Men skyldnerne, skærer dem op og fjerner de nye organer. Ud af dette uhyggelige og undertrykkende samfund træder den unge Shilo, som lider af en sjælden sygdom. For at finde kuren må hun bevæge sig ind i GeneCos bizarre og mørke verden, hvor hun ikke blot kan finde nøglen til sin fremtid, men også risikerer en voldsom og pinefuld død.
