Biutiful er historien om Uxbal (Javier Bardem) og hans kærlighed til sine børn. Han arbejder i Barcalonas underverden, hvor han importerer illegale kinesiske immigranter. I hverdagens uskyldige verden er han fokuseret på at være en kærlig far for sine to børn, men da han en dag får stillet en uafvendelig diagnose må han nu forholde sig til sin egen dødelighed og forsone sig med alle tilværelsens mørke sider. Samtidigt forsøger Uxbal at redde sine to børns tilværelse, inden han for evigt opsluges af det mørke, der har været hans ledestjerne.
