در میانه یک گشت معمولی، افسر دنیل کارتر با چهرهای خونآلود که بهسختی در حال لنگیدن است، در جادهای خلوت برخورد میکند. او جوان را به یک بیمارستان روستایی نزدیک که کارکنان کمی دارد میبرد، اما بهزودی متوجه میشود که بیماران و پرسنل در حال تبدیل شدن به موجوداتی غیرانسانی هستند. همینطور که ترسوهراس بیشتر میشود، کارتر سایر بازماندگان را در سفری جهنمی به اعماق زیرزمینی بیمارستان هدایت میکند تا در تلاشی ناامیدانه قبل از اینکه خیلی دیر شود به این کابوس پایان دهد.
