Selepas virus misteri menghapuskan sembilan puluh peratus manusia, pontianak muncul dari ceruk bumi untuk memperhambakan manusia, melayan mereka seperti ternakan. Selepas bosan menderma darah sebagai pertukaran untuk perlindungan, Yuichiro Hyakuya yang berusia 12 tahun dan sahabatnya Mikaela Hyakuya merancang untuk melarikan diri bersama anak-anak yatim piatu yang lain.
